Your Next Step After Cardiac Rehabilitation
Continue your wellness journey with confidence. Stay active, connected, safe, and supported at the Y.
Healthy Hearts is designed for individuals who have completed cardiac rehabilitation and want continued encouragement, activity, and connection.
Through partnership with PeaceHealth, cardiac rehabilitation graduates can continue their wellness journey at the Y with resources designed to help them feel confident, safe, and supported.
You choose what works best for you. Use your YMCA membership independently, schedule a Wellness Welcome Session, participate in Healthy Hearts community activities, or combine the options that fit your goals.
Disclaimer: This program does not replace the critical role that medically supervised cardiac rehabilitation plays in recovery from a cardiac event. Healthy Hearts activities are not medically monitored sessions.
Try the Y and see how it supports your healthy lifestyle.

“The chronic disease prevention programming is a strong motivational force for getting up in the morning and getting out of the house. The physical exercise program is excellent along with the camaraderie with all who participate. The attention given by the staff and volunteers is excellent.”
